It does not look like a codec problem to my inexperienced eye. The AVI itself seems to be corrupt or to contain incompatible elements. You may have to go to the source and get or make a new avi, watching the production process very closely, or questioning the provider about the source closely.
The references to the "splitter", and the 0 length references are red flags, expecially since codec issues are not red flagged by the analytical tools you used.
